Objective
The student
-is able to assess the resources and risk factors related to the health, living conditions and life situation of the elderly and the need for service and rehabilitation based on them in a client-oriented manner.
-is able to act as an expert in nursing in multidisciplinary RAI activities and is able to utilize multidisciplinary knowledge in the individual planning of the care and service and rehabilitation needs of the elderly.
-can guide the elderly to find ways to survive at home and remain functional (multisectoral services).
-is able to carry out a wide-ranging proactive home visit and direct it to the necessary services.
Content
Promoting and assessing the health and functional capacity of the elderly people in health care work. Identifying resources and risk factors and supporting health and functional capacity, creating opportunities for inclusion and empowerment. Health screenings and multi-sectoral services for the elderly and the specific features of health counseling. Introduction to the RAI system. Opportunities and use of welfare technology. Proactive large-scale home visit.
