Objective
Students will learn to describe case management as an approach to work, its principles and processes and to analyse levels and methods in case management. Students will learn to describe in theory what case management involves, including the assessment of clients' life situation and service needs, counselling and advice, needs assessment, planning and implementation of services. Students can theoretically explain how to counsel, advise and support older people and their families to help them meet individual needs, cope with daily activities and solve problems related to wellbeing, functional capacity or life control. Students know how to use public, private and community services and support to promote their clients' wellbeing and daily coping. Students understand the coordination role of the case manager in planning, implementation and follow-up of services designed to meet client needs.

Grade 5:
Students have extensive, in-depth knowledge of the contents as defined in learning outcomes. They competently examine and analyse principles of case management and extensively apply the theory of case management. Students analyse future challenges in case management, giving profound and extensive arguments.
Grade 3:
Students have good knowledge of the contents as defined in learning outcomes. They apply the theory and models of case management in practical assignments. Students analyse future challenges in case management, giving appropriate arguments.
Grade 1:
Students have basic knowledge of the contents as defined in learning outcomes, but their use of the knowledge is limited.
They can to some extent examine and apply the theory of case management.
Qualifications
previous studies are not required
